Initial Nutrition Assessment - Online

Service Description

A comprehensive nutrition assessment and discussion of current issues and goals. You will leave this initial consultation with some strategies and resources to support your first steps towards positive change. Once you have booked, you will be emailed a Pre-Consultation Form and a Three-Day Food Diary to complete prior to the consultation. Please allow enough time to complete the Food Diary and return it at least 48 hours prior to our meeting. If you can't find these forms in your inbox, please check your junk/spam folders and add us to your contacts! Payment by internet banking on receipt of invoice. NOTE: cancellation within 48 hours of appointment / no-show at appointment will incur a $25 fee. Booking of this service is acceptance of this condition.

Cancellation Policy

For cancellations, please contact me at least 48 hours prior to avoid a cancellation fee of $25 (for consultations or workplace sessions).
